Holiday Closure Notice: Monday 5th May 2025
Please note that our practice will be closed during the May bank holiday on the following date:
- Monday 5th May 2025 (Early May bank holiday)
To ensure you have an uninterrupted supply of medication, kindly submit your medication requests no later than 12:00 PM on Friday 2nd May 2025
In case of urgent medical assistance during our closure, please dial 111.
For emergencies, dial 999 immediately.
Online Forms
Online forms are available Monday to Friday, 7:00am to 11:00am and 1:00pm to 5:00pm
Due to the high demand for Appointments we have to change the opening times and closing times in order for us to process all forms clinically appropiate.
At times the online forms will not be available due to demand exceeding our maximum daily capacity.
Patients can walk call Monday to Friday between 2pm - 4pm using extension 8 if you require support to complete the online form.
Staff will be happy to assist you to complete the forms or during our less busy periods.
